Nested Care Group (“we”, “our”, or “us”) operates in Western Australia providing registered NDIS disability supports, clinical nursing, allied health, and Home Care Package aged care services. We strictly respect the privacy of individuals who interact with us and are bound by the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (Cth), as well as the NDIS Quality and Safeguards Commission rules.
2. Information We Collect
To deliver tailored, safe, and clinically governed care, we may collect personal and sensitive health information, including:
- ✓Full name, date of birth, contact details, and residential address in WA.
- ✓NDIS participant number, plan management details, and funding budgets.
- ✓Clinical histories, mobility assessments, medication charts, and PBS plans.
- ✓Emergency contact, guardian, nominee, or support coordinator contact details.
3. How Your Information Is Used & Shared
We only collect and use personal data for purposes directly related to your care:
• Service Delivery: Designing and implementing individualized care plans, clinical nursing triage, roster scheduling, and goal tracking.
• Clinical & Safety Governance: Ensuring Registered Nurses and support workers have the necessary medical instructions and emergency protocols.
• Regulatory Compliance: Reporting as mandated by the NDIS Quality and Safeguards Commission, Department of Health, and Aged Care Quality Standards.
• Zero Commercial Sale: We will never sell, rent, or trade your personal data to third parties for marketing purposes under any circumstance.
4. Data Security & Retention
All digital records are securely hosted in enterprise-grade, encrypted Australian data centres complying with ISO 27001 and Australian privacy protocols. Access is strictly restricted on a need-to-know basis to authorized clinical and coordination staff.
Participant records are retained for statutory periods in accordance with the NDIS Act and WA Health Records retention guidelines, after which they are permanently and securely destroyed.
